Coordinated Care Initiative
California’s dual eligible integrated care demonstration is called Cal MediConnect and is one component of California’s Coordinated Care Initiative (CCI). The CCI is a new state law that changes the delivery of Medi-Cal, LTSS, and Medicare for dual eligibles and seniors and persons with disabilities living in eight California counties. The timeline for the CCI differs county by county.
This website is designed to provide information and education materials regarding the Coordinated Care Initiative. NSCLC does not provide direct services to beneficiaries.
If you are a beneficiary who received a notice about Cal MediConnect or changes to your health care benefits, please contact the Health Insurance Counseling & Advocacy Program (HICAP) at 1-800-434-0222 or Health Care Options at 1-844-580-7272.
